This information applies to U15/U16/U17/U18 divisions.

U13 and U21 may book appointments, but it is not covered in CRAA fees and will be billed back to the family.

This testing is REQUIRED for participation this season in minor aged body checking divisions  - although we are well aware that it may take some time to get in. Please book ASAP!  Managers will be checking that this has been completed by all team members by the end of October.

There will be a large volume of athletes completing baseline assessments in September / October from the new participants, so please note it is best to book earlier versus later. Athletes can expect to be at BCI for up to 1.5 hours to complete all the necessary assessments.

2024 Instructions

Hello athletes,

Thank you for choosing the Benson Concussion Institute as your trusted medical team for the upcoming season.

We recently updated our online baseline assessment appointment booking. Please carefully read the instructions for athletes to our BCI High-Performance Concussion Program.

Step 1:
Please note that the booking system is not compatible with Internet Explorer or Microsoft Edge browsers.  Please use Google Chrome, Firefox or Safari.
Use this link to start: https://portal.healthmyself.net/bciconcussion/guest/booking/form/97f1bd8f-7862-4081-b74b-6ce05e56bdcd

Step 2:
Choose Returning Athlete or New Athlete.

If you have completed a baseline assessment with BCI before, you are considered a Returning Athlete. Welcome back!

If you have NEVER completed a baseline assessment with BCI before, you are considered a New Athlete. We look forward to serving your brain health and performance medicine needs this season! Please note that a comprehensive pre-season healthy baseline assessment assists our clinical team to tailor medical decisions and management recommendations to your unique characteristics.

Step 3:
You will then need to fill out the Registration form. Please click on the terms of use and privacy policy links before you accept the terms and conditions.

Please note that athletes can only register their accounts using a unique email address. If you receive an error message that says your email address is in use, it is likely you have a registered/unregistered Pomelo patient account at a different clinic, or a family member has your email address in their account. Please complete registration using an alternate email address or contact the Pomelo Patient Support Team through the "Need Help" link at the bottom of the registration form.

Step 4:
Note the name of your organization, complete the required E-forms, and confirm the email address you would like to use for appointment related notifications.

New Athletes please allow about 10 minutes to complete these forms so choose a time when you have no distractions. These forms must be completed in one sitting.

Once your appointment is confirmed, please create an account with Pomelo Platform to book future appointments. If you already have an account from the previous year, simply login and follow the instructions.

You will receive a confirmation email once you have submitted your appointment request with your appointment details. There is a link in this confirmation for the Consent form (You can find this attached in the email as well). This is the only form you need to bring with you to your baseline assessment.

Our new system will also email you an appointment reminder before your baseline assessment.

We have also added a pdf with a baseline preparation checklist

If you have any questions about how to access your account to schedule a baseline assessment, please call the clinic at (587) 391-9840 or email info@bciconcussion.ca

Thank you,

The BCI Team
